Privacy Statement
For proper treatment, it is necessary that I, as your treating therapist, create a file. This is also a legal requirement under the WGBO. Your file contains notes about your state of health and information about any examinations, diagnoses and treatments carried out.
Your file also contains information that is necessary for your treatment and that I have requested from another healthcare provider — for example, your GP — but only after receiving your explicit consent.
I do everything I can to safeguard your privacy. This means, among other things, that I handle your personal and medical data with care, ensure that unauthorised persons cannot access your information, and that I, as your treating therapist, am the only person with access to your file.
I am bound by a legal duty of confidentiality (professional secrecy).
The information in your file may also be used for the following purposes:
- to inform other healthcare providers, for example when therapy has been completed or when referring you to another practitioner. This is done only with your explicit consent.
- to inform a colleague providing cover during my absence, if necessary. This is done only with your explicit consent.
- for anonymised use during intercollegiate consultation.
- some information from your file is used for financial administration/bookkeeping, so that I or my administrator/accountant can prepare an invoice.
If I wish to use your information for any other purpose, I will inform you first and explicitly request your consent.
The information in the client file is kept for 20 years, as required by law. You have the right to view, amend or request the deletion of the information I hold about you.
